Very very general bug that may apply to the main game, but if someone is equipping an item right as everyone gets on the elevator, one item that is unequipped will be highlighted as equipped in the inventory. I was scrambling around trying to equip the right item just as my party went down from the lobby, so I'm not sure if I even selected the highlighted-but-not-equipped item.
