T1 items dropping in T2 (and on?)

I've never had this happen to me until recently, and this is the second time now:

I opened a chest and a T1 healing capsule fell out. The same thing happened when I killed a Chromalisk in T2 a week ago.

To be honest I've only seen it happen on D9, maybe D10.
