Like you, I too work in the software industry. I understand that things happen during development, testing, deployment and while in production. However the idea is to document the issue, cause(s), steps to reproduce and the resolution in order to learn from the mistake; whether hardware, network and/or software related. Taking into account today's and previous months updates, I'm left with questions and suggestions.
Do you all quality check your work?
Today's update, along with previous updates have been released with horrible bugs and server-side issues. Do you all have testing and staging servers that you all perform mock deployments to and test all features/bug fixes? If so, then you all need to evaluate your current QA and deployment procedures because today's buggy release is a prime example that the answer to both questions is no.
Suggestions:
1. Thoroughly quality check all work within the current iteration.
2. Perform aggressive regression & unit testing.
3. Evaluate your current testing/QA procedures and streamline them.
4. Perform mock deployments on testing and staging servers.
5. You all should be the very first to be posting a status update thread here and in-game on what issues are known post-update.
6. Stop using your production environment as your staging/QA box. (It feels very much like this is the case.)

If this is true I wonder how safe Spiral Knights really is...
I'm pretty sure they may check some quality, but as you pointed out, they could do better.
