I was so unlucky that I was the only one in a party of four to not get a book before Nick ever even mentioned that the book was only supposed to go to one person.
I was in a party with a guy; after dying, we agreed that it would be simplest if he just ran for the party pad and revived me there. The kat spawned just before the party pad, and he managed to revive me, but the kat was stuck on the other side. Eventually managed to kill it by blowing up Biohazard charges I tagged on it, but still couldn't get the tokens, and didn't think the GMs would appreciate being called out for such menial duties. They get enough stuff to deal with on a regular basis anyway.
When it happened to me, it was the first Kat I saw out of the gate. Then I accidentally killed him at the start gate... Gm would have come, but it was after midnight when I gave up waiting. Then at ~1:00 they responded that they would come grab the tokens for me... A nice gesture, and much appreciated, but I was long gone at that point.
EDIT: I think Dukeplatypus had a book drop behind a gate... ouch.
@Balderus: That sucks. Even though it was never meant to be, that is very painful. Have you gotten a book yet?
