The bugman is a phantom variant that's yellow and very tall and has a hood. He carries a scythe and acts like a phantom.
His level would be a revamped graveyard, where zombies are replaced by 10ish grave scarabs each.
The bugman would smell your fear and trigger monster gates blocking your progress in the hallways and edges of arenas. You would need to kill him 2-3 times per level to pass, at least. Yes, he comes back like a phantom.
He is called the bugman because when he does the projectile spawning attack, he summons grave scarabs instead of bullets.
