So I was in a random challenge room while on a t1 run (so I could get rid of the crown cost in t2) when all of a sudden, this happened:

Graphical Sword Bug

I experience this fairly often, although never to that severity. My Divine Avenger shrinks temporarily until I swing it again, sometimes a little, and sometimes a lot. It doesn't seem to effect the hitbox or anything, just a graphical bug.
