Your screenshots look very similar to what I saw when my graphics card was failing. I didn't know what was happening at the time (for months). Finally my computer crashed and wouldn't reboot. The boot-up process would no longer make the single beep it normally does to indicate detection of a working graphics card.
The desktop PC was less than a year old, and I sent in the graphics card for warranty replacement. I had to use my old PC for over a month while waiting for the replacement part. I received no updates indicating Cyberpower would send it any time soon (or ever), so I finally sent an email begging and pleading for the part. Shortly after that, they finally sent it (ATI Radeon HD 5850).
The new card has worked ever since installing it, except that the audio from the HDMI output quit working for a few months. The audio made a horrid noise for a few seconds, then there was no more sound. Windows list of playback devices then showed a red circle with a line through it on the AMD HDMI Output. It remained that way until it mysteriously started working again a few months later. (The video from that HDMI continued working the whole time.)
