Could somebody explain how to get a Forge Prize Box? All I know is it has something to do with the forge.

When you forge an item that is 3 star or higher using the maximum number of Fire Crystals you have a chance of receiving a forge box.
This box contains:
Fire crystals of the same kind used to get the box
Orbs of the same star level as your item
A Variant ticket
Sometimes it has:
Confetti (often)
Shadow lair key (rarely)

Ever! lol. When it began I got three after about 300 forges; now, more than 3,000 forges later...zero.

By the way, here are my personal data:
85 Forgings total, all at 100%. (I made most of my equipment before the Forge was added to the game.)
1 Forge Prize Box (from forging a 5-star item), with the following contents.
* 25 Radiant Fire Crystals
* 5 Sparks of Life
* 1 2-UV Punch ticket
* 1 4-star Orb of Alchemy

Based on what you've shown me so far, it seems that it may be around 1 to .5% but is higher and lower depending on what star rank it is. Does anyone else know anything about the percentage?

Within the ones linked in my study via Bopp, I've had 5 forge prize boxes recorded [3 from me personally] pointing to an average close to 1 box every 250 forges.
I have an odd feeling that forge prize boxes are related to lockboxes in how they are obtained.
Friends have gotten free forge boxes while crafting shortly after opening multiple bought ones.
I don't have any data to support this claim. All my personal data in my study are without bought forge prize boxes.
You have to use the 100% chance forge choice, and there is a very small chance of getting one. Its not usually something thats farmed after, its usually just a very pleasant surprise.
