i found it in a treasure box at jelly place near the core of ruby gate
sorry i will update with a link later on stats if peopole are interested.
and maybe someone put it in wiki cause im to lazy to do that :)

Level 1 abilities?
Haha, you have me blocked in-game, I see :P
I see a +4 health bonus... Do you remember if there was still a +4 health bonus when it was level 1? Since it's a five-star item, I would assume it is....

Saphy, only at lvl 5 the
Saphy, only at lvl 5 the health bonus is increased. This goes for every item that I know off, at lvl 5 and 10 they have the option to gain an attribute.

It does somewhat look like an xelor doesn't it.
Cyanide purchased the recipe for this earlier today, so it's not necessarily an ultra-rare item. But the art is nice.
