I just finish running T3 OCH, but I didn't get any mask fragment. Did I miss some area?
Or am I supposed to finish T1 and T2 first?

its weird, its supposed you get the fragment in the prize box room (after defeat seerus and getting down to the elevator). the fragment always spawn just right side to the broken seerus mask. sure you didnt pick up and its on your inventory now?. because since the fragment its gray color its always camouflage with gray floor.

I don't have any fragment in my inventory... I guess I didn't pick it up then... :/
At the very end, it'll be on the floor similarly to how shadow lair mats work. After beating seerus, just keep a lookout for it!
