Fast-swinging swords will regularly "skip" attack animations, in that the client doesn't see the swing occur, and/or the monster being attacked is not affected by the swing (no damage numbers, no knockback on last swing in combo). The skipped animation might be completely omitted, or only part of the animation might be shown. The character will almost always NOT move forward on skipped sword swings.
This is especially notable with sowrds at Maximum IAS, but is not limited to the fastest Cutter swords, either. A Brandish sword (all variants up to and including the Combuster) will also have their attack swings skipped with regularity.
No performance issues are apparent with Spiral Knights on my computer, and this occurred with both the latest 1.5 JRE and the current 1.6.0_26 JRE after a reinstall of SK in between changing the JRE.
While the damage lost is a royal pain, even moreso is the lack of knockback and interruption on end-of-combo attacks that SHOULD be affecting monsters, except that the server appears to simply ignore the fact that the monsters are immediately adjacent (or even in a corner, at times) to the player, a complete sword combo is performed (rapid-clicking, not counting the clicks out one at a time), and one of the sword swings, often the last, is simply not performed as it should be.
Resolving this bug would be greatly appreciated.

I usually account it to connection lag. I've noticed this since I started playing from time to time.
