It happens only since latest update and in under specific conditions, which are:
-after I use a charge attack
-after I get hit
It doesn't happen all the time, though its extremely annoying. My Divine avenger combo can't execute, meaning that no matter if I click with the right timing, my character will keep doing only the first it of the combo, until I move elsewhere. Its pretty disturbing, the 2nd swing of the Divine Avenger is required for me.

It is a bug because instead of it being: 1 > 1 > 2 like it should, it is: 1 > 1 > 1 > 2. For whatever reason that second swing does not allow you to make a full combo. Waiting for a moment before attacking fixes the problem, but when you're in combat you don't always have the luxury of waiting.

Added note: There is also a bug with the Flourish line (at least the Furious Flamberge, as that is what I have): If you swing once, shield, and then try to combo you will do a second first swing and then finish with the final combo lunge.

This bug is bothering me.
This bug is bothering me. It's exists for both the avenger and faust, and it happens when you
Attack 1
Shield Cancel
immediately Attack 1 again
Cannot perform attack 2 under these circumstances.
This bug has been around muuuch longer than the latest patch. It can also be triggered by shielding before the first swing's animation has finished.
