How does CTR stack?

I have a few questions on how CTR bonuses stack together.:

1) If you get a CTR:Low bonus piece of armor, like a Demo Suit, and combine it with a bomb that has a CTR:Low UV, does that equal CTR medium, or some unspecified value in-between?

2) Is it possible to exceed CTR:Very High? That is, if I get a CTR:VH UV on a bomb, and then put on a demo suit, is there going to be an increase or is the reduction capped?

3)Last, If I get something with a CTR:low or medium UV, and then add heat, will it increase to medium/high and then high/very high at levels 5 and 10? That doesn't seem likely, since it would make Very High UV's less valuable, but I wanted to check and see.

The UV qualities equate to

The UV qualities equate to numeric values. Add together any identical qualities to get the final modifier.

1 | Low
2 | Medium
3 | High
4 | Very High
5 | Ultra
6 | Maximum!

So if you're using, say, two pieces Wolver plus Bristling Buckler with a Calibur that has UV - Sword Damage Bonus: Medium, you have

Sword Damage Bonus : (1+1+1+2 = 5) Ultra.

Keep in mind you can't get beyond 6, so there's little purpose in using, say, two pieces Vog Cub plus a Switstrike Buckler with a sword that has an Attack Speed Increased UV.

Also keep in mind this applies to penalties, so if you're using Spiral Plate Mail plus Spiral Plate Helm plus Swiftstrike Buckler, you'll have Attack Speed Increased: Low.
