For the record, I play on linux and I never experience lag other than the one caused by my own local network traffic, so it's not a linux issue.
Please, how do you intend to address the issue of lag?
Most of the lag I experience now is lag coming off of my hard drive when it goes to randomly load something. I never had this issue on my SSD. Whatever it is that's loading, it's probably using a lot more memory than it needs. Diablo II had this exact problem in a few areas. Duriel, and baal come to mind. By the time everyone's PC was done loading, at least one player would be dead! This was very unfun for hardcore players.
Otherwise the once in a while hiccup from internet lag comes and goes. I haven't experienced it yet today or yesterday, but I'm waiting!
So whats a good ping? Never heard of it before this thread. I checked mine, said 250.
According to pingtest.net, I have, between myself in Northern Ontario and some server in San Fransisco, CA...
A ping of 96ms, jitter of 4ms, and a packet loss of 0%.
Play was completely impossible on the very first floor we stepped on.

I play on a Mac, although I really hate them. My mom bought one for college, although she really doesn't use it for that anymore. My internet is 56mbps, and my ping ranges from 33-150, so I have extremely good internet. Thing is though, the only reason I probably lag is because I play on a laptop. Yeah, that pretty much takes the cake for the reason I lag. I'm going to build a PC soon, with very good parts, tons of HD space, RAM, and good cards. Hopefully after this, I won't lag whatsoever. What's also weird is that, even with my good internet, the game says I only have 3 bars on the top right corner. It could be a problem with me using a laptop, the game still registering all the new people that are playing, or the servers themselves.
Yes I have this sort of "latency lag" wherby monsters between me ( client-sided ) and the server ( server-sided )
Signs of latency lag :
1. You raised your shield at the time the monster attacks and still get hit.
2. 1 with traps
3. switches activate a little later ( about 0.3seconds for me)
4. lag but no botching around ( teleporting )
I managed to deal this by acting more aware and be ready at any time so i wont get hit, the game is still great

When you get to the pingtest.net step, be sure to choose a server near or in California for a more accurate ping. This will allow for geographical distance.
I've made every change I can, but I still experience terrible lag.
I submitted a support ticket, and apparently there was nothing wrong with the server side. I'll try again the next time this happens, which I fully expect to be the next time I play. :/
I'm not griping, I'm just getting a more realistic view of my circumstances.
I live in Southern Ontario, have a ping of 78ms, jitter of 5ms, packet loss couldn't be measured and can play perfectly. Are you using a satellite connection? Have you tried updating your wireless card drivers? Also consider updating your graphics card drivers. Well consider updating all the drivers on your computer to ensure they are all running the very latest version.
Try running pingtest in firefox to check for packet loss. Chrome won't do it.
My drivers are okay. I've enabled QOS on my router. Nobody is torrenting.
So here's an interesting situation.
Before SK I tested my ping (to California). 0% packet loss, ~90ms ping, ~4ms jitter.
While running SK, that jumped up to a whopping 1551ms ping and 2291ms jitter. Still no packet loss.
Oh, now it's back down to 130ms/16ms. The game is unplayable. I don't know if I'm alive or dead, here or there. Things float through walls, die, stand up again, jump back and forth...
Nothing changed on my end. SK is running just fine, SK is totally unbearable, SK is running just fine.
120/8 is the best I've seen, with three computers connecting wirelessly. The game is fun and playable. For now.
This is what makes me think that my drivers are alright.
Then, disaster strikes and I have a ridiculous ping of 1551/2291. That's got to be a measurement error, right?
What should I do?
I would have to assume that's a measurement error. For a reference here's my current pingtest. I ran it at various times during different play times and that is a rough "average" of what I see. I'll see that ping anywhere ± that jitter. Any test at the same, roughly 2000 mile, distance nets the same result. (I even tried to Greenland :P)
It's a slow night on the internet, but it's uncommon for me to see less than 100/20 (Ping/Jitter) at that distance. The in game meter sticks around 3 bars average, seeing 5 or 2 frequently. Even still, I do not get rubber band attacks. There's some minor player position correction going on and a bit of delay, but the game is still entirely playable. I can still weave T3 gun puppies with reasonable accuracy (still getting random micro corrections and being hit sparingly).
I've had this happen. It usually happens when there's too many things on screen at once. Suddenly things will seem to lag and everything in the room will randomly teleport all over the screen. Your character will adapt a "jumping" behavior of travel. Playing the game like this is impossible.
The solution: Put graphics settings on low. Go to advanced video options and turn on compatibility mode. The problem should stop.
The other possible problem could be your internet connection. If it's your internet connection then at first it should seem like nothing is wrong, but enemies should stop moving and your attacks won't do anything. Occasionally your knight will teleport across the room. If it's your internet connection then you need to contact your ISP and work with them.

Having an unreliable connection doesn't mean your drivers are fine. There could have been a driver update to fix that unreliability, you never know.
@Zaroni It does happen when there are too many things on the screen, but it also happens when all of the monsters are dead and we're running to the elevator, and anywhere in between.

I usually only experience the "lag" described by most players in this thread when I have left my computer on for too long and it is either overheating or the memory leaks have built up. Usually a quick restart guarantees me a couple hours of lag free gaming. So the problem seems to be more client-side rather than server side.
