I hope it doesn't involve luck or lag. Is there a trick to consistently getting:
1. 3 hits on a gun puppy?
2. 3 hits on an enemy that's cornered or walled?
3. 2 hits on an enemy in the open?
...with one charged attack?

If you freeze a target (cryo gun, any level of the shivermist bomb or freeze vial) and get close to the target before unleashing the charged attack, it will hit 3 times.
As for gun puppies, try facing them before doing so. It should hit three times.
To hit them more than once without any status effect, the target either has to be really slow (like a Lumber) or it has to be next to something indestructible like a wall or solid block. Shield bump them into those or just try to target the ones who are near those and wait/lure those who aren't into the strategic spot.
Again, if you're not facing them, you probably won't get 3 hits out of the charge.
Hmm... I personally don't find it's as easy as facing the puppy. Do I have to start the attack point blank? Because I have "autoaim" on, so I should always be facing the victim upon letting go of the charge. I thought there's an angle to it and I have to disable "autoaim" so I can face slightly to the left or right... (or should I disable "autoaim" because the server thinks I'm facing another direction...)
As for mobile enemies and walls, doing it not on a corner wall is largely inconsistent for me, but a real corner seems to work very well.
If you are behind it then you can hit it while it's turning then go around it and hit the puppy.one of the best tactics for puppy's I saw so far.
agreed with above for gun puppies, just go next to it and charge attack.
for anything else i tend to hit them from SSW or SSE of them, seems to get the 2 hits in.

I find that Calibur's first hit to have a wider reach than the second and third hit.
