Do you need to complete all 15 levels in ONE time ''without'' going to haven to get tier 2???

No. That's not even possible

No. That's not even possible since there's only 6 levels (I assume basil doesn't actually count) between the start and Moorcraft (aka Tier 2). You just have to complete 15 levels. Any kind, anywhere.
