Im planning on getting a Fallen Armor, but Im not really sure what does the "penalty vs fiends med" really mean.
Does it mean that I deal less dmg to fiends? or does it mean fiends deal more dmg to me? or is it something else?

you do less damage to fiends.
you do less damage to fiends. So, if you are wearing both pieces and have sword damage Max!, you'll only do sword damage Medium against fiends, and Max! against everything else.

its like a bonus
but does that much less damage
dmg inc vs fiend max = lots of dmg vs fiends
dmg pen vs fiends max = barely any damage vs fiends
but yeah, what nordlead said.
