I've noticed that Punch only gives defense UV's on shields and never resists, yet you can get a resist UV on a shield while crafting. Is there any other gear that this kind of thing is true for? Also, it seems that Three Rings could make a lot of money by selling custom ordered Unique Variants, so why aren't they doing it?

Because
Custom-ordered Unique Variants would turn this game into one of those cash-shop MMOs where P2Ps get all the optimal gear if they cough up.
You can buy good UVs from people who get them, but the key is that it all depends on luck.
