If a piece of armour with an accessory on it is used for crafting, is the accessory removed, and thus destroyed?

Yes, but.
They stay as long as that slot stays on the armor. The common example is that a long feather would stay from Cobalt to Solid, but Mighty's plume would eat it.
Just check the line you are going to put it on before you do to make sure it has the same slots.
Accesories stay.
