When alchemizing a new item from scratch, I had the "keep unique variants" checkbox present which had stuck around from looking at another known recipe that I didn't alchemize.
Was a Spiral Demo Suit I was making so that box shouldn't have been present.
It happens on the standard server, too. So probably should just be a bug report.
