When I look at a guildmate and he's in the dungeons it says a location like B19F.
I get that the 19F means 19th floor but what does the B mean? Is it ever not a B?

. . . .I'm preeeetty sure that B stands for "Below" . "Basement 19F" Doesn't make all that much sense, but "Below 19F" makes at least a little ;) .
EDIT also BTW, to answer your second question: the B never changes to something else, it's always B.
