Right now, I'm wearing a Sinister Skelly suit, and both my helmet and suit are level 7. To "evolve" into 5* weapon, I just need 3 levels left (yay!). However, I have a predicament... I'm wondering, that if I add an accessory to my sinister skelly suit now, will it carry onto the 5* version? Or will the accessory be destroyed upon transmuting? Any help is appreciated! Thanks!

If the accessory you put onto an armor/helm can be put onto the armor/helm that it "evolves" into, then yes.
So if the "evolved" armor does not accept the accessory, then it will get destroyed. Example: Ashtail Coat with any aura will lose its aura when turned into Skolver Coat
Any accessory on Sinister Skelly Suit will be kept when turned into Dread Skelly Suit.

The accessory will be attached to the upgraded item, yes, except in a few cases, in which the upgraded item does not have the accessory slot needed to hold the accessory.
In your specific case, you can examine Sinister Skelly Suit and Dread Skelly Suit on the wiki, and see that they both have the same four accessory slots. So there is no danger of your losing your accessory. The same goes for the masks.
