Currently, only items have been given out for free based on your rank.
During Homecoming, some shiny pieces of 5* gear were given out for players at Vanguard, with other * gears for appropriate ranks. The Guild Hall update also featured Trophies based on your rank.
But that's all judged by your rank, not your Prestige numbers. So, strictly speaking, they do absolutely nothing except a badge, which only goes up to 45K Prestige.

Right now, just a shiny badge next to your name. There may be more to it in the future, but we haven't heard anything about it recently.
