If you crash, you may get disconnected from the server. Kicks dont send you to haven, only kick you to a copy of the same level, alone.
It's also possible they just finished the level so obviously you would be sent to haven since the battle is over.

Before making OOO responsible, why don't you find the reasons of the crash. Could be faulty drivers, java itself, another program clogging your system, you usign a cheap computer.
It's not like everyone's crashing all the time to be the game's poor coding the only thing to blame. And no one will help you with that attitude.
Most probably the rest of your party went in the elevator before you tried to get back in. If you had logged in fast enough, you would have appeared in IMF.
