Your eye color will always stay the same, and some armors, especially magic robe-style armors (Stripe on the top of head, trim on robes) will change color based on your color. My vog cub is practically the same as anyone else's though, so it really depends. Shields have no color matching (to my knowledge).

Design Color only affects your starting armor's colors, as far as I can tell.
Personal Color affects your eyes, your nametag, your minimap dot, and certain highlights on all armor sets you equip. (i.e. the sleeves on the Spiral Demo Suit, the skirt on the Valkyrie Mail, the pigtails on the Firebreak Helm).
Shields are not considered part of your armor, so the colors on your shield are not affected.
Starting color absolutely affects what your gear looks like. Look at any two people wearing identical armor. On the Wolver Coat, for example, your gloves, the inside of your upper arms, and the outside of your thighs are all you-colored. With the Wolver Cap, the side and back of the hat's "ears" are you-colored.
EDIT: Oh, I'm dumb. I'm talking about personal color, everyone else is talking about design color.
Design color should be applied to more armors, imho. Not sure what the point is of customizing a two-color scheme and then never seeing it once you get 'real' gear.
This is like when you choose hairstyle in any other MMORPG, once you get a helm, you wont see the hair anyway..
Some people choose to wear their newbie helmet or armor as costume just to look different from everyone wearing the current wolver line uniform. I do this myself - never thought I'd end up wearing ponytails, but looking like everyone else in Vog Cub gets a bit old after a while.
So that's the only reason to pay attention to the design color. Personal color on the other hand affects a lot of things, as the posts above point out, so you should give some thought to that because you can't change it later.

Design Color is mostly so you don't look exactly the same as a third of the people at the Rescue Camp, nor your teammates in the dungeon that leads to Haven.
Yes. Most or all (my experience is limited, so I don't want to promise it's all) armors and helmets have portions of the design that change color to match your personal color. The Heavy Demo helm, for instance, has a stripe down the center that will match your eyes. The spiral plate mail, looking at the image on the wiki, seems to have bits by the hands and on the back that will match your personal color.
