It has indeed been suggested before, and should be suggested again, and as many times as is necessary to get the job done. One slight change: like the dapper combo, I would suggest separate top hat and monocle accessories, as well as a new combined helm top accessory that contains both top hat and monocle--that way with both the new combo and dapper combo you could rock the full top hat/monocle/stache/pipe.
[slight edit: and let me just add, the 3* kat hoods would look AMAZING with the aforementioned combination of top hat/monocle/stache/pipe...]

Before all of these cosmetic things you want, have you ever thought of enabling Eye Accessory and Mouth Accessory first? I mean... this should be done before bringing MOAR and MOAR cosmetics...
And I like how most people want cosmetic suggestion more than game-improvement thingies...

An amazing concept, good sir. However, there is the imminent possibility of so-called "clipping" for the Top Hat. On headgear such as plate and pith helms it would not be so bad, as it could rest on top of those. However, what of helms such as any cobalt aside from Azure, and I am laughing my head off thinking of how it would fit on Dread Skelly Masks. Perhaps it would be limited to certain helms. I am assuming that it would be helmet top, that would make it easier to limit.

Although I probably would never wear a monocle and moustache, I still have to vote an obligatory +1 to having monocles and having two separate slots - Lower Face and Eyes.
Pretty sure this has been suggested before, but that doesn't make it any less awesome.
