But how often/rare does this happen? Is there a known percentage for weapon drops? When was the last time a free piece of equipment popped out of a box for you?

I got a emberbreak armor when I was 2* (practically 10 months ago), then I never saw an item drop again. I would say it's quite rare.

I would say you get one roughly every 200 hours. That's about my rate for it.

A few weeks ago I got a Big Beast Basher and a Jelly Helm from two of the boxes after we beat JK. And it was in one run. I wish I'd find a Darkfang Shield when I do T3 Clockwork runs though e.e'

I got some... 5 drops? But that was 2* crap.
Also in the topic of drops lemme warn the egoists: After kicking all rare drops disapear. Always check, that party is closed, rly.

He sold it and used that CE to help get him to T3.

I got a Skolver Cap in T3 during The Silent Legion. Rolled on it and got Piercing Max and sold it because why not.
Also, I'd have to say the dorprate for Items is roughly about 0.1% along those lines.

680 hours played on my main, three item drops.
- 2* Jelly mail (boxes post-Snarbolax boss fight, I think)
- 1* Scale Shield (Treasure Vault)
- 3* Scary Skelly Shield (left path's red boxes between statues and zombie arena in FSC D26)
Which dropped in that order.

I gotten about 7 free stuff.
I noticed that T1 has better chance of dropping 1* stuff. Found 2 shock edges, an icebreaker shield (i think), and scale helm.
In T2, i got 4* Ash Coat from RJP.
In T3, i gotten 4* Skelly Suit, and a 4* Skelly Shield from FSC
and i remember playing with a friend and he got a 4* Ash of Agni drop
My playing time: 47:00:14:37 what ever that means.

I tend to run into a drop weapon/armor about every 6 to 8 weeks on average. Last thing was 2 days ago; a wise owlite shield.

2 Thwack Hammers in one danger room's treasure boxes (I still have no idea how that happened, but I gave them both away to other party members), 2 Crystal Bombs on separate RJP runs (pre RSS change), and recently a Skelly Suit in the LoA treasure room. This is with a time value of 40:16:30:56, whatever that means. It seems I am doomed to get nothing better than a 2* equipment drop... although I did get a Book of Dark Rituals, so I guess it balances out.
A friend of mine claimed he had a Gran Faust drop from a box, which was why he was selling his old Faust off. I can't verify if that was actually true or not, though.

PvE guild doing too much FSC to even count how much per day or even week.
Usually an item drops every 2 weeks or so.
5* are rare but they do drop, seen Ash of Agni and Magma Driver drop...
As well as Graviton Vortex, Graviton Bomb, Firo Driver, Strike Needle and many... many more.
The past 4 months has been raining items for our guild master... :D

Used to be easy to get an item drop back in those days. And now ye can get 2* stuffs even in Charred Courts easily while the sneaky dumb 5* stuffs are in 'dream on' mode.
