Mission, stratum, level, or whatever you have encountered.
Personally, I think its the T3 Operation Crimson Hammer.
I have no idea how I did that 3 times to get the mask.

I honestly think shadow firestorm citadel isn't so bad compared to other areas. Curse doesn't really do anything if you have weapon slots, and is way not as bad as shock/fire. I'd say that without not less than max+max+low resistance for immunity, C42 is the hardest. After that, the first level of the red roarmulus twins (provided you don't know whats ahead of you).
UFSC and C42 are easy :P
I'd say a depth 28 shock fiend danger room would be the hardest challenge SK has to offer.
Return of Ur, Gauntlet, And all-fiend stratums.
UIMF. Compound 42 doesn't require a fee to enter, so you can eventually memorize the spawns and get in position to assassinate polyps/menders before they become a problem. On the other hand, I've only been through UIMF once and am never returning willingly.
C42 is only bad if you aren't prepared to resist fire and have to actually deal with it. It becomes the worst after that.
Yeah, I have the dragon scale set with poison and fire trinkets, and C42 becomes a cakewalk as long as you're not affected by the slime boogers on the floor.
Oh god...Shock fiend arena T3 depth 28 at the last wave...they cannot be stopped. Get your asi max flourish, ctr max DBB, or ASI max gun, and try your best. But once you get shocked once, its over.
Besides that absolutely terrible experience, those levels with the minis where you have to get 2 keys to get to elevator are annoying as heck. The slime ones are hard to not die from, the bite range of the beast minis are insane, and the shadow ones catch you off guard. Worst part is, they give no rewards ;(
any Stratum 6 shock fiend arena, danger room, etc is significantly harder than anything else (but also way more fun)
Fun? As long as you are running on mist D;
Forgot to mention oiler danger rooms with fire pits. Firewall of death, no escape. :(
Any Fiend level. HoI takes the cake for Freeze but just like people said, Shock is a complete NIGHTMARE. One shock and you are dead, period.
But in my opinion, out of every non-Fiend themed area. Darkfang Munitions Factory is by far the hardest, even more then Rabid Wildwoods and Ice Queen Palace. (Darkfire Citadel was a pushover though, more like a breather dungeon)
The amount of times i wiped there i cannot even to think of, Shock and Fire is just a pain dumb combo. You are getting shocked randomly and constantly taking damage from Fire. Shock resist is a MUST for this place unless you are a complete beast, know the layout and spawns by heart.
If i ever get around to going to that place again, i am so bringing Vog set with max Shock resist, if i ever get it that is. The chance of me going back there though is somewhat likely as i have plans to solo them with no deaths some point in the future, much like Vokster did.
I did solo Darkfire Citadel once (Got in for free because some random guy opened it while i was looking for recipes, everyone but me rage left the first level but i kept going. I still got screenshots on my Steam account of me downing Darkfire Vanaduke and reaching the Sanctuary) but died seven times in the progress.
FSC, UFSC and C42 if you're doing them for the first time.
For the first time? Red roarmulus twins was awful for us the first time back when it came out.
Any Fiend stratum or tier 2/3 turret bullet hell, both with a status, along with any Deconstruction Zones, depending on the layout.
For missions... err... anything with fiends and that danger mission with the dead things. And the Gauntlet.
1. Red Roarmulus Twins: Pretty hard if you don't have someone that isn't familiar with it. Did it 10 times by now, all without wipes, except for the first one.
2. C42: It's only hard if you don't have fire resistance to the oil drops. Poison is annoying too, but bearable, unlike the fire which wipes most of the parties.
3. Heart of Ice (Before the devilities nerf): THIS MISSION WAS THE FREAKING HARDEST MISSION FOR ME WHEN IT CAME. THIS MISSION CASTS A SLEEPY SPELL ON ME, THEREFORE, I CAN'T FINISH THIS MISSION WITHOUT FALLING ASLEEP ONCE OR TWICE.
4. Gremlins Arena T3 depth 28 if possible: This turns out to be MUCH harder than Fiends when Thwackers gang on you with their hammers and the healers remove their injures non-stop. It's really fun, and gives some challenge.
5. Some Danger Rooms in T3: Fiend DR are pretty challenging, even more with shock or fire status. Undead is lame, Jellies give some challenge while in groups, so does Gremlins. Beasts are lame as always.
Recently I've been finding Heart of Ice almost impossible to solo, just because of the lost souls that spawn against small parties. They respawn so quickly that there is no point in killing them. And there are so many of them, and their freeze is so strong, that I'm frozen all the time, even though I'm wearing at least freeze+7. Darn you, freeze armor.
Were lost souls added sometime after the mission was created? I don't remember this mission's being so difficult.
Ghosts in the Machine also has lost souls with that nasty shock status, but I don't find that mission hard to solo.
Not the hardest but I can see people having problems with Terminal Meltdown's last part.
The most difficult I've encountered was T3 OCH while suffering from lag, especially during that one fight with the Battle Pod with the switches which alternate the doors keeping Gremlin Knockers and Gremlin Thwackers at bay. If anyone on the team isn't a team player and keeps hitting those switches, the entire team can get swarmed by Gremlins from all angles. The Menders don't help much.
Without lag? A swarm of Devilite Overtimers. They're pretty much the only monsters I resort to spamming Polaris shots at.

Not for the difficulty of Monsters,
Not for the Boss fight.
But for the crazy amount of Laggin Outs & Crashing that you go through on a regular basis.
Me and 2 friends have attempted a run, and crashed 14 times between us, that was just the 1st 2 levels.
By far the longest and most grueling area in the game as far as I know, and is filled to the brim with curse, the deadliest status. Add in lots of server latency and liberal use of fire and spike traps, and respawning howlitzers, and the area is as hard as it comes. Add the fact that Carnavons (curse zombies) are invincible from behind and can inflict curse with their breath, and it makes waves a lot slower as well. Darkfire Vanaduke also has an absurd amount of health, cannot be shivered, and deals curse with every attack. The unknown passage, ironically, is the easiest part of that particular shadow lair by most accounts.
