I have over the past week experienced issues several times where either through game crashes or ISP issues I have had the game closed and/or lost connection with the server.
This often happens every now and then and is usally not a problem. Your knight is still left in-game and assuming you restart the game/re-login you will be connected to it again.
The bug/issue in this case is when that DOESN'T happen. I'm using steam so it auto-logs me in as soon as the game starts up again and it should be starting to connect me to my knight. However, the game has either flat out refused and crashed at that section at times over the past week for me, or, in the case of today, it has gone so far as to put my knight name up in the top left corner of the window; And then proceeding to get stuck in loading hell. However, even in the most recent cases knights in my party have mentioned that while the D/C logo above my head went away for a short while, it came back a min or two later while I was still stuck on the "logging on..." screen.
The boiling point for me was really today when the game crashed in the middle of a shadow lair run and I couldn't reconnect to my knight no matter how long I waited or how many times I tried. Not untill the knight was fully removed from the game could I login again. Thankfully a GM helped me out and threw me into the party again. However, near the end of that floor I got latency issues with my ISP (And let's face it, not everyone have ideal ISPs.) and ended up stuck in limbo. My party members were walking all over outside the void and everything else was frozen. (Clicking the spark of life usage button does nothing, by the by.) Oddly enough, the voice chat functionallity still seemed to work just fine as I could hear the only party member using a mic talking and stuff. I was in this position for at least 10mins, and occasionaly I would see things that had happend for the party members quite some time ago; This effect only lasted one or two secs before being frozen again.
So, being stuck in limbo forever is not really ideal, I killed the java process. (Since that counts as a crash rather than a quit in SK, making the knight still be there.) and started up the game again. Yet surprisingly, I can't connect to my knight. Yes, yes, you may proceed and call me stupid for thinking something would work when it did not work last time, however, this is a game functionallity that have worked perfectly in the past. (As far as I know.) And dare I say it's one of the most critical game functionalities we have.
At that point I just gave up and stopped trying. (And I'm sorta paronid about not wanting to bother GM about things, especially since I already did a short while ago with the same issue!) And I know, I know. You don't need my life story for a bug report, but this is the sort of thing that happens when there are bug and issues, and I wanted to share that insight and story. If you need any clarification on anything I will be able to supply that tomorrow! :V

Try this method when you get disconnected again:
-Log in using a different account.
-Select your knight.
-Once you reach the Ready Room, log off.
-Log in again using the D/C'd account.
-The game should load normally now.
