I've heard that danger mission difficult scales with your gear, so it will always present a consistent challenge. Is that true?

Yep. When a friend or guild mate of yours is doing a danger mission, check them out in your Social menu, to see what depth they're on. If you have a wide variety of friends/guildies, then you will see a wide variety of depths, even if they're all doing Legion of Almire (for example).
Not really with your gear, but rather with your rank.
When you first unlock the prestige missions (rank 4 or 5) they are at T2 difficulty. As you level up to Vanguard, they end up being on T3, at stratum 6.
