The uv ticket can be for a single uv, a double uv, or a triple uv(unsure about this one).

My rewards
Ive got and opened one radiant forge box.
Got sparks, radiant fire crystals, a couple orbs (dont recall what type), 3UV ticket, and a shadow lair key - all bound. (So had to do the SL myself instead of selling.)

Are you saying that it's impossible to get a Shadow Key in a Forge Prize Box? Why do you say that? Is there is some official announcement of the contents? I'd like to see it, so that we can improve the wiki.

About the contents
I'm hoping to add a list of the contents with a study I'm hosting to add to the wiki knowledge. Unfortunately, I haven't gotten any more boxes yet (nor anyone helping with the study either). I've heard from other people the SL key is "rare", which by OOO terms means ~1% chance.

Theyre way to rare
Ive never met someone whos gotten more than one forge box, i think everyone only ever gets one, or one a year.

-Varying Fire Crystals (I think number is affected by Variant Ticket)
-Variant Ticket, either 1, 2, or 3
-Sparks of Life
-Chance of Orb
-No keys, but chances are it's possible to get
-I might have gotten a slot? Forgot. Don't trust me on this one.
I'm fairly certain chances of getting 1, 2, or 3 variants are the same.
