Sorry if this isn't very helpful, but this was a long, long time ago and I can't remember all of the details too well.
I am certain about the item and the mission, but the rest of the information listed I'm just pretty sure about.

Also I got a 2* wolver helm along the way, I believe it was t2, like right after you leave moorcraft, and i think it was a wasteworks? Sorry for the lack of remembering.
I am one hundred percent positive these happened, I was partying with others for them both.
