I was in the mission where you put a sword in a dead knight's grave , after the 4-trojan fight , I went to the elevator while the party was smashing stuff , I decide to jerk off , the party came back to the elevator and at the same time one of the lost souls killed me (i had half a pip of health).
Alll that beautiful heat....gone....

WOOPS
Mender pushed me into spikes with his AOE heal (Stay close!).
It was the third battle of an arena level, last wave, T3.
The mender was the only mob left, aside from some thwacker who was hiding in a corner due to low health.
I only had 2 hp when this happened.

same thing happened to me only the mender was the only enemy left apart from a mecha knight in its death animation.
My biggest fail was when I died to a green jelly mini in royal jelly palace.

So here's the story.
This actually happened today. So I only had 349cr left and I was out of energy. I decided to play some T2 LD. I had my gear all on and I was set. I put on my Swift Flourish. I didn't have any T2 gun at the moment so I equipped my Proto gun. Better than nothing, right? So I went to go play LD. When the game loaded up, I talked my little before-match conversation and noticed I had my proto gun out. I hit the spacebar to switch my weapon and guess what?
Nothing happened.
I did it again, and again, and again. It wouldn't work. I checked my arsenal and I found out some devastating news. I had accidentally clicked "Equip as Weapon One" when I equipped my gun. So the only weapon I had on was my proto gun. Of course, the other team was full dusker.
I tried and tried with my proto gun and I must have done like 35 shots on this one guy before I actually killed him. Then 3 more of his team members came over. And I died.
Then, when the match ended, I looked at my damage; the least I had ever done: 324. My team lost.
I told everyone that I was just gonna go equip my Swift Flourish then come back. So I clicked "Exit." I equipped my fast sword and was ready to go. I clicked on the Dual Swords and was ready to hit "Play" when the button was gray. I wondered, "Why is it gray?"
Then I saw my crowns.

Also, got killed by mini jellies at least 3 times in a single JK run, I blame lag.

FSC Charred Court, the room with the rotating Fire bar. Electron Vortex. I stood next to the switch on the side where the Zombies spawn and hit the switch with a Vortex.
And I didn't move out of the way.

I was going to bid on this featured volcanic dragon wing on AH while it was cheap, but all I had was 20kce.
So I transferred it to cr...but when I checked the Ah it has gone another million..then I exchanged it back to cr losing some amount of ce =[

Some failing.
And here's something that happened to me. So what' going on? We are at the last phase of the Vanaduke battle, and I don't notice all my party mates are dead. I'm about to strike a blitz charge, when Vanaduke does that charge attack and hits me. I die, but the blitz charge kills Vana. Result: both us and Vana are dead. A giant token falls from the sky, with us being unable to pick it up.
Aww, who's reviving now?
And that happened to me two times.

arrogance hurts
I was playing t2 LD today, and I was at half a bar of health. The match had nothing but noobs so I got all arrogant and egotistic. I took a seat on a point saying to myself I'm such a bad*** there's no way I'm gonna die, besides there's no ones around. 5 ****ing seconds later I get hit by a proto sword. Then the noob started bragging, and I start raging. Note to self never let your guard down.

so that is how i died in t1 with a full 5* set ( volc. plate mail [sleep weakness, whoops] and dragon scale helm [piercing resist ToT] ) due to sleep and piercing attacks from a single lichen.

MY worst fail so far
Well, my worst fail so far would have to be when A friend needed help to go through one more elevator. I came to him at the cost of 10 energy. The plan was simple, we just needed to open a tab for him to complete the mission. When I got to where he was, I entered the elevator and took over his mission... I forgot to open a tab. He was sent to the haven. It was pretty bad.
@happyapathy in particular: That is why I use my laptop's pad and not a mouse. I feel for ya though, did that on my first JK... -___(\
To you who've posted. I've laughed and realized I'm not failing so bad.
Biggest fail thus far:
Soloing JK using Firecracker and my Troika (indeed I was that stupid)
Charging firecracker and actually setting JK on fire often enough, I got him to final stage. Charge one last time (hopefully the last hit in a long, long session) on my troika (pre-patch) and get killed. JK still dies due to fire status.
I had no friends to invite... (still extremely noobish) no guild.... and worst of all... a 20 me rev. Guess my mist count at this time? NINETEEN.
Timeout on the return to haven... guess my mist count after the screen loads...
